POGO Automatic is an easy-to-handle, all-in-one, discreet blood glucose monitor that helps break down barriers to help patients achieve their glucose goals.

How to Prescribe for Medicare Patients

Follow these steps (to view more information click on the (+) symbol):

To see if your patient is covered under Medicare Fee-for-Service, refer to the CMS Glucose Monitors coverage criteria

  • POGO Automatic Monitor is classified as code E2104 
  • POGO Automatic Test Cartridges are classified as code A4271 

Your patient must meet the following criteria to be covered under Medicare for a POGO Automatic:

You have concluded that the beneficiary has: 

  • Diabetes (Refer to the ICD-10 code list).
  • Sufficient training using the device as evidenced by providing a prescription for the appropriate supplies and frequency of blood glucose testing.

Intuity Medical also suggests an explanation of why the beneficiary has an impairment of manual dexterity that requires they use this automatic blood glucose monitoring system​

  • i.e., the beneficiary has challenges handling a traditional blood glucose meter, testing supplies and/or test strips and lancets (as certified by the treating practitioner as supported by documentation in the medical record).​

You can provide a written prescription or an e-prescription via your e-prescribing software. In your software, search for “POGO Automatic”, the POGO Automatic Monitor and Test Cartridges will appear in the drop-down menu. Select quantity and the number of refills. 

You can submit your Medicare patient’s e-prescription to either retail or online pharmacies. 

  • Retail pharmacies – We are actively growing our list of available pharmacies, currently only Kroger pharmacies can fulfill Pogo, or
  • Our online pharmacy partner – HealthHavenRxTM can fulfill Pogo. HealthHavenRx is a secured and convenient way for your patients to use their health insurance and get free delivery.

Note: For option #2 above, when sending to our online pharmacy partner, you can find HealthHavenRx in your e-prescribing software as: Health Haven Pharmacy, Nashua, NH. Can’t find?

Complete a Standard Written Order Form and send this order and the patient’s most recent medical records, demonstrating all coverage guide criteria are met, to a pharmacy as noted above in Step 2.

How to Prescribe for Private Health Insurance Patients

You can provide a written prescription or an e-prescription via your e-prescribing software. In your software, search for “POGO Automatic”, the POGO Automatic Monitor and Test Cartridges will appear in the drop-down menu. Select quantity and the number of refills.

Option 1: Your patients’ preferred local pharmacy; check out the extensive list of pharmacies.

Option 2: Our online pharmacy partner, HealthHavenRxTM, a secured and convenient way for your patients to use their health insurance and get free delivery.
