POGO Automatic ® Subscription Program Terms and Conditions
Intuity Medical, Inc. (“Intuity"), the maker of POGO Automatic, now offers a special discount if you subscribe for POGO Automatic® Cartridges (“Products”) today.
Read terms carefully as they may be subject to change.
For subscribing, you will receive a discount on your recurring POGO Automatic® Cartridge orders (the “Subscription Program”) on the online POGO Store. POGO Automatic Cartridges will be automatically delivered to the address you specify based on quantity and delivery frequency you select. POGO Automatic monitor required and must be purchased separately. Discount applies to POGO Store only and will not be honored at your pharmacy. This offer cannot be combined with other offers.
Subscription discounts and other limited time promotions may be found on our website presspogo.com (the “Website"). We reserve the right to change the Subscription Program benefits, including the discount amounts and eligibility used to determine discount amounts, at any time at our sole discretion. We will give you notice thirty (30) days before any changes are put into effect. If you do not agree with these changes, you may cancel your subscription under this Subscription Program in that notice period. Changes will be applied to future orders following the notice period.
Shipping
Free shipping for orders in the U.S. within the 48 contiguous states, Washington D.C. and the Armed Forces (U.S. Postal Service). Products are not shipped to Hawaii, Alaska, or Puerto Rico. If you are located in any of these areas, check with your local retail pharmacist to obtain your POGO Automatic Monitor and Cartridges.
Subscriptions automatically renew unless cancelled by you. By purchasing a Subscription, you acknowledge that it has recurring payment features and you accept responsibility for all recurring payment obligations prior to cancellation of your Subscription.
By providing us with information in order to avail yourself of this offer, you are giving us (or an authorized person on our behalf) permission to charge your payment method, plus any applicable taxes and shipping. For subscriptions, your payment method will be billed at or just prior to each shipment.
Price Changes or Cancellation
At any time and at Intuity’s sole discretion, Intuity may cancel this program, or change the pricing of any Intuity Product or Subscription. We will notify you thirty (30) days in advance for changes in Subscription pricing. If you do not cancel your subscription during the notice period, new pricing will be reflected on your then next billing statement. Be sure to check your junk folder to make sure you do not overlook an email communication from us.
Customer Cancellation
You may change or cancel your Subscription through your POGO Store account or by contacting Intuity Customer Support at 1-855-IMI-POGO (464-7646). If you wish to cancel your Subscription, you must do so prior to your next payment charge date. If you cancel your subscription and then reactivate it, the discount may have changed in the interim, and a new discount may be applied to future orders.
Purchases Prohibited FOR RESALE
You acknowledge and agree that Intuity Products purchased are for your personal use and you acknowledge that you may not resell any Intuity Products to any third party, and shall not be sold to any other person, entity, posted for sale or distribution elsewhere, public or private. Failure to comply with this section or any other part of these terms may result in Intuity, in its sole discretion, taking such actions as it deems appropriate, which could include, actions such as suspending your account with or without advance notice and without liability to Intuity.
Future Promotions or Offers
From time to time, Intuity may offer additional promotions or discounts, for which additional terms may apply and which will be provided to you at that time. You must provide, or have, a valid payment method on file in order to receive the benefits of the Subscription.
